Permission Slips — Staff
Day trips and overnight trips are separate programmes, not variants of one.
Turn-in status
"Not yet" does not always mean not yet. This list is built from
recorded consents. A slip submitted through the public tournament form sits as an
intake row until an Admin reconciles it, and reconciliation happens on the Admin
Portal, not here. Until then that family shows as not yet even though they
submitted. For a tournament programme, check the intake queue before chasing anyone.

This list shows whether a slip exists, not what it says. Medical notes, signatures and answers are not read here — they are on the slip itself, which opens in the viewer.
Authoring is blocked (BDL-199). Creating, copying, superseding and
activating templates are switched off on this page until that ticket clears. The
controls below are visible and filled in so the shape can be reviewed, but nothing
will write. Clear the block by setting AUTHORING_BLOCKED to false in this page's
source once the registry says so.

Required ticks come from a fixed list. Every
ack
must use one of these seven keys, because the returning-family shortcut and
has_consent match on the spelling:
A new required tick — an overnight undertaking, say — is a schema change:
the list has to be extended first. An optional tick needs
nothing: make it a flag with any key you like. Choices are
election, also free-keyed, with two or more options of at most
60 characters.

Fill in the trip details. Student name and school are not asked for here — they differ per family and are filled in when each guardian signs.
Everything you type here — trip details and statement wording — appears on
every family's copy of this slip. No participant's name belongs in any of it.
